Include your name, address, phone number and email address; a description of the removed content and where it appeared; a statement under penalty of perjury that you believe in good faith that it was removed by mistake or misidentification; your consent to the jurisdiction of the courts for your address; and your physical or electronic signature. We will forward the counter-notice to the person who filed the original notice, and may restore the content if they do not take legal action.

We may limit or end access to our website for anyone who repeatedly infringes the rights of others.
